Diagnosis and Treatment of Specific Disorders - Menopause and Peri-menopause: diagnostic testing would include a Pap smear, mammogram, blood work. A vaginal ultrasound may be done, and possibly a bone-density screening test. Treatment would include emphasis on diet, exercise, calcium, plus use of natural hormones, bone strengthening medications, and possible natural products and/or supplements available without a prescription.
- Abnormal bleeding: We obtain a careful history, try to elicit details about the amount of bleeding, the timing, possible associated symptoms. Then after the exam, there may be a vaginal ultrasound, possible endometrial biopsy (now almost painless!), possioble intrauterine saline hysterogram (a "salt-water" ultrasound), possible surgical D&C, with diagnostic videohysteroscopy. Treatment might be just reassurance, use of birth control pills or other hormones. Advanced treatments include operative videohysteroscopy, intrauterine surgery, endometrial ablation and/or hysterectomy.
- Management of PMS: use of menstrual symptom calendar, history of lifestyle issues, verification of diagnosis versus other emotional disorder, hormonal testing. Treatment may be: use of targeted medications, lifestyle modifications, vitamins and natural supplements, exercise program, referral to other specialists.
- Evaluation of abnormal breast lump. Initial history looks for risk factors for benign versus malignant disease. Cysts can be aspirated in the doctor's office. Referral to radiology for mammogram, possible breast ultrasound, possible 3-d stereotactic guided needle biopsy. Possible referral to surgeon for excisional biopsy.
- Abnormal pap smear evaluation: history of abnormal pap smears, vaginal infections, careful exam to look for causes, colposcopy exam using magnification of the cervix, possible cervical biopsies. Treatment may include cryotherapy (freezing) of the cervix, possible wire loop electrosurgical excision (called LEEP), possible cone biopsy, possible laser vaporization.
